项目摘要

非アルコール性脂肪性肝疾患（NAFLD）はメタボリックシンドローム（MetS）の肝病変であり、炎症・線維化を呈する非アルコール性肝炎（NASH）への進行は予後不良となる。ナトリウム利尿ペプチド（NP）は心臓由来のホルモンで、抗炎症・抗線維化作用を有するが、肝病変に対する応用研究は少ない。本研究ではNAFLDにおけるNPの役割と治療応用について解明することを目的とする。2022年度の臨床研究では、外来通院中のNAFLD患者の肝臓の病態及び毛中利尿ペプチドレベルの検討を行うため、NAFLD患者の前向き研究を開始した。その他疾患の利尿ペプチド系への影響を除外するために、除外基準としてウィルス性や自己免疫性肝炎、慢性炎症性疾患や悪性腫瘍の併存としたため、今年度対象となった入院患者はいなかった。そこで外来患者を中心に研究参加を募り、参加患者の情報を前向きに収集している（UMIN-CTR＃000048569）。また、NAFLDにおける利尿ペプチド系の肝臓局所における関与を確認するために、市販されている健常者及びNAFLD患者肝の組織切片を購入し、H＆E染色、Masson Trichrome染色及びナトリウム利尿ペプチド受容体の免疫染色を行った。NAFLD肝では線維化・脂肪化を認め、ナトリウム利尿ペプチド受容体の発現も確認した。2023年度は引き続きNAFLD患者の収集に努め、腹部エコーによる肝脂肪化及び線維化と利尿ペプチド系バイオマーカー濃度測定を行いこれらの関連について一次解析を行う予定である。また、遅れている動物実験に関して実験環境が整い次第再開し、組織解析による病態生理の解明を行う予定である。

Non-alcoholic fatty liver disease (NAFLD) is a disease of liver disease, inflammation, and linear maintenance. Non-alcoholic hepatitis (NASH) is a disease of liver disease. Diuretic (NP) has been studied for its anti-inflammatory and anti-vascular effects on heart disease. The purpose of this study is to clarify the treatment of NAFLD. The 2022 clinical study begins with a review of liver disease and diuretic disorders in patients with NAFLD in general hospitals. Other diseases include diuresis, chronic inflammatory diseases, and chronic ulcers. The Center for Foreign Patient Study Enrolment and Patient Information Forward Collection (UMIN-CTR#000048569) Liver tissue sections from healthy individuals and NAFLD patients were obtained and H&E staining, Masson Trichrome staining, and immunostaining were performed to identify the relationship between diuretic status and liver function. NAFLD liver fibrosis and fatty tissue development is confirmed. In 2023, the collection of NAFLD patients, abdominal fat, liver fat, and diuresis were determined. The animal's physiological state is regulated by the environment, and the pathological physiology is explained by the tissue analysis.
