超高齢社会のわが国で、認知症患者に対する義歯補綴治療は、既に高頻度診療行為の一部である。認知症者は慣れた義歯の使用は可能でも新製義歯への適応が困難であるとして、現義歯の調整や修理を優先すべきとの指針が示されているが、義歯新製以外に選択肢がない場合に、予知性を疑問視しつつ新製に着手することもしばしばあり、認知症高齢者に対する有床義歯補綴の予知性向上は、本邦歯科補綴学の喫緊の課題である。本研究は、歯科訪問診療において新規に有床義歯（全部床義歯もしくは遊離端サドルを具えた部分床義歯とし、欠損形態によらずインプラントオーバーデンチャーは除外する）を製作する65歳以上の全症例について、義歯製作の着手時点で評価した有床義歯補綴治療の難易度（日本補綴歯科学会編「補綴歯科治療の難易度を測定するプロトコル」JPS Version 3.1）、義歯非装着時の口腔機能（最大咬合力、咀嚼能力、唾液分泌能）、ならびに認知症の諸症状が、新義歯装着後3カ月後の装着状況（義歯使用有無）に及ぼす影響を調査するものである。このうち認知症症状に関しては、中核症状とADL障害をDASC-21にて、またBPSDの精神症状と行動症状を、それぞれ日本語版NPIならびにDBD短縮版で評価し、中核症状、ADL障害、BPSDのいずれが義歯使用有無と関連するかを検討することで、認知症高齢者に対する有床義歯治療の予知性向上を図ることが狙いである。歯科医療者のみで実施する本研究においては、認知症症状の評価に関して十分な信頼性を確保することがきわめて重要な課題である。そこで初年度は、本研究への参加の意思を表明した14名の歯科医師がDASC-21のe-ラーニングの受講を終えるなどして、認知症症状の評価の経験を重ねた。加えて、少数の臨床例で試行的に認知症評価を含むすべての評価項目の記録を実施し、歯科訪問診療における義歯製作と並行して、これらの記録が可能であることを確認した。

The treatment of dental prostheses in patients with cognitive disorders is part of the high-frequency diagnosis and treatment. The symptoms of cognitive disorder are related to DASC-21, BPSD mental symptoms and behavioral symptoms, Japanese version of NPI and DBD shortened version, nuclear symptoms, ADL disorders, BPSD mental symptoms and behavioral symptoms. This study is an important topic for dental practitioners to assess cognitive symptoms and ensure confidence. In the first year of the study, 14 dentists participated in the study, and the evaluation of cognitive symptoms was reviewed. In addition, a few clinical trials of cognitive evaluation include the implementation of records of evaluation items, dental visits, and the preparation of records.
