项目摘要

本研究は、染色体や妊孕性の問題に直面する性分化疾患（Disorders/Differences of sex development：DSD）を伴う原発性無月経の女性を対象に、倫理的配慮が整った診療環境の整備および診療場面での心理的ケアの実践に向けた、看護師向け診療援助のケアガイドの開発を計画している。今年度は、これまでの研究プロセスから医療従事者の情報の全面開示（full disclosure：FD）と支援に関する対応の実態を解明することを目指し、他施設との共同研究を実施した。研究対象は、染色体46,XYを伴うDSDである完全型アンドロゲン不応症（complete androgen insensitivity syndrome：CAIS）18名とした。CAISは、性同一性（gender identity）は原則として女性であるが、疾患説明には、精巣の存在、不妊、性染色体XYなどの項目が必須となることから、患者の受ける動揺が大きいと推察できる。調査は、医師側の立場から後方視的にデータを収集し、疾患説明の現状を記述的および数量的に検証した。医師側の説明時の工夫や配慮を明らかにすることで看護実践における課題も明確になると考えた。その結果、このような調査は日本では初であり、説明表現は症例ごとに個別化した言葉を選択しておりCAIS患者への疾患説明の模範例として有用であることの示唆を得ることができた。一方で、説明時に看護師が関与したケースは件数としても限られている現状が明らかとなった。従って、DSD医療において有益であるとされている多職種によるチームアプローチに看護師が積極的に関与する必要性が課題として明確となった。
