This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The purpose of this pilot study is to test the ability of diet plus exercise to deplete intramuscular lipids (IML) in the absence of weight-loss, to assess the influence of IML on whole-body insulin sensitivity by measuring insulin sensitivity before and after IML depletion, and to identify any racial differences in IML depletion or insulin sensitivity changes. To assess this, 12 healthy, sedentary men (6 Black and 6 White, pair-matched for age and body mass index) between the ages of 19 and 45 will consume a very low-fat (less than 10% of daily calories), moderate-fiber (>25 grams/day), isocaloric diet for one week. The subjects will undergo measurement of body anthropometics, body composition analysis using DXA, insulin sensitivity testing, and blood draws (for fasting serum cholesterol, HDL, LDL, triglyceride, free fatty acids, leptin, and adiponectin levels) at baseline and after 7 days of controlled diet and exercise. The hypotheses of this study are that a low-fat diet plus exercise will deplete IML in the absence of weight-loss, that depletion of IML will have a positive effect on insulin sensitivity that is independent of weight loss, and that there will be racial differences in IML resulting in differences in insulin sensitivity changes.
