METRONOMIC DOSING OF CYCLOPHOSPHAMIDE AND THROMBOSPONDIN LEVELS IN IDIOPATHIC NE

The objective of this study is to evaluate the association between thrombospondin-1 (TSP) levels and response to metronomic dose cyclophosphamide (CTX) in patients with Nephrotic Syndrome (NS). Specific Aim: To determine whether TSP-1 levels in patients with NS correlate with response to therapy with CTX. In this pilot study, we will obtain serum from subjects with steroid resistant, steroid dependent, or frequently relapsing NS who are about to initiate a 12 week course of CTX. We will obtain serum prior to initation of CTX, a second time at the midpoint, and at the the completion of the 12 week course. Our hypothesis is that subjects with Nephrotic Syndrome who achieve remission with CTX will show increases in their levels of TSP, suggesting that the beneficial effect of CTX is mediated by TSP-1.
