This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. A pilot with implications for community health, the purpose of this study is to develop an educational intervention program to improve the quality of diabetes care. Primary care physicians, who receive continuing diabetes medical education, will be encouraged to refer patients to the study from their diabetes practice. Referred patients will be invited to participate in the intervention program. Some participating patients will be trained as peer motivators and others will receive mentoring on how to better manage their diabetes while a third group will provide the necessary controls.
