This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This is a study which will look at how three different beta-blockers compare in effecting change in the structural, hormonal and cellular response of the heart, in patients who have "dilated" congestive heart failure. Patients will be randomly chosen to take either one of three types of medication. Various tests, such as echocardiogram, heart catheterization and nuclear tests, will be taken at baseline, at 3 months, and at 12 months. There is no "sugar pill", or placebo and patients will know what medication they are taking. The study lasts 18 months. Patients will be kept in the GCRC for one to two nights. Also, patients may participate in a substudy, which will look at how the heart responds to certain intravenous heart medications by measuring the cardiac output and pressure readings of the heart.
