Health technology company Mybesthelper Solutions Inc. has developed a personal care platform called Careteam, that patients, their families and caregivers can use to coordinate care for patients at home. Their platform enables healthcare providers, the patient and family to access a care plan through a single mobile and web location. Their customers are health care organizations who are responsible for the cost of caring for patients recovering at home following discharge from hospital, in the transition to community care or those aging at home. There is no existing equivalent that ties the formal discharge process and the post-discharge process together in a single information and communication platform.To support the design and implementation of Mybesthelper's Careteam platform, research is needed to understanding the barriers and facilitators to patient adoption and engagement. For this project the Health Design Lab at Emily Carr University will lead a human-centred design research study to gain an understanding of the factors leading to the adoption of the care platform, the factors liming access to the platform and the user experience (UX) and design factors that could lead to improved adoption and engagement. The goal of this project is to increase user adoption of the Careteam platform in order to improve patient outcomes. By providing better coordinate care and patient support Mybesthelper seeks to reduce: patient length of stay in hospital (LOS), readmissions to hospital, discharged patients presenting at the Emergency Room, missed appointments for treatments, and health care delivery costs.
