我国是全球精神分裂症的第一大国,由其导致的疾病负担给我国社会经济发展带来了沉重负担。WHO,UNESCO等国际组织推荐将社区康复作为防控精神障碍的重要策略。国家卫健委、民政部也多次发文要求加快发展精神障碍社区康复服务,其中明确提出“重点突破一批基础性、关键性、实用性康复技术”。在此背景下,本研究拟在“社区融合”框架下,综合运用流行病学、数理统计学、卫生管理学等理论和方法,开展精神分裂症社区康复适宜技术的遴选、优化与评价研究。首先,在“社区融合”的框架下,对社区康复技术进行初选。其次,在供方、需方、技术和环境视角下,应用灰色层次分析模型和模糊综合评价遴选社区康复适宜技术。再次,通过多属性决策模型等方法优化适宜技术方案。最后,应用社区干预实验评价和比较适宜技术效果,形成可推广的精神分裂症社区康复适宜技术目录和方案。该研究对于我国发展社区康复服务,防控精神分裂症具有非常重要的理论意义和实践意义。
英文摘要
China ranks first for patients with schizophrenia in the world, and the burden of schizophrenia deeply influences the development of social economics. Community-Based Rehabilitation (CBR) as an effective strategy on schizophrenia was recommended by the World Health Organization (WHO) and other international organizations. In China, several documents were released by National Health Commission and Ministry of Civil Affairs to accelerate the development of CBR for the mental disorders. It also clearly mentioned that “building several fundamental, key and practical rehabilitation technologies.” In the context of these, we research on the selection, optimizing and assessment of the appropriate technologies about community-based rehabilitation for schizophrenia. The methods in epidemiology, mathematical statistics, and health management were used under the community integration framework. Firstly, we want to select rehabilitation technologies under the community integration framework. Secondly, we plan to select the appropriate technologies about community-based rehabilitation for schizophrenia under the consideration of providers, consumers, technologies and the environments. Thirdly, we want to optimize the project of appropriate technologies with the methods that expert consultation and multi-attribute decision model. Finally, community intervention experiment will be used to evaluate and compare the effect of these appropriate technologies, and we will also build the propagable list and scheme about the appropriate technologies. We think this study has nice theoretical and practical significances on the development of CBR and the control of schizophrenia in China.
