精神分裂症(SZ)异质性造成的治疗困境是亟待解决的问题。有创伤经历的SZ可能是一种独特的亚群,创伤经历与该群体更差的神经发育缺陷、精神症状、治疗效应和功能结局密切相关。既往研究提示眼动脱敏与再加工(EMDR)疗法适用于伴创伤症状的精神病患者,SZ患者存在组织和整合与事件关联信息能力的缺损,改良EMDR则可能改善伴创伤SZ患者的创伤症状、疾病严重程度及功能结局。由于SZ患者情景记忆和想象上都存在具体细节的提取障碍,本项目拟构建创伤场景库和美好安全场景库,采用图片激发的方式协助患者实现稳定化和创伤回忆。针对有创伤症状的患者,在常规药物治疗稳定3个月后的继续治疗下,随机分为传统EMDR治疗组、改良EMDR治疗组及对照组,考察改良EMDR疗法的有效性和安全性,随访验证疗效的稳定性;同时,采用多模态磁共振技术,解析EMDR起效的神经环路机制并构建疗效预测模型,从而为SZ患者的治疗优化提供理论依据。

The treatment dilemma caused by the heterogeneity of schizophrenia is an urgent problem to be solved. Schizophrenia with traumatic experience may be a unique subpopulation, and traumatic experiences are closely related to poorer neurodevelopmental deficits, psychiatric symptoms, therapeutic effects, and functional outcomes in this subpopulation. Previous studies have suggested that eye movement desensitization and reprocessing (EMDR) therapy is applicable to psychiatric patients with trauma-related symptoms, but patients with schizophrenia have deficits in the ability to organize and integrate event-related information. Modified EMDR may improve trauma-related symptoms, severity of disease and functional outcomes in schizophrenia with traumatic symptoms. Due to the deficits in specific details of situational memory and future imagination among patients with schizophrenia, our project will construct a traumatic scene library and a beautiful security scene library, using pictures to stimulate patients to achieve stabilization and traumatic recall for EMDR therapy. Patients with traumatic symptoms will be randomly divided into traditional EMDR treatment group, modified EMDR treatment group and control group under the continuous treatment of stable drug treatment for 3 months. The effectiveness and safety of modified EMDR therapy will be investigated. We will follow-up to observe the stability of the treatment effect; at the same time, the multi-modal magnetic resonance technology will be used to analyze the neural circuit mechanism of EMDR and construct a curative effect prediction model, which provides a theoretical basis for the treatment optimization of patients with schizophrenia.
