外科术后肠动力障碍(POI)是腹部手术常见并发症,也是影响患者术后康复的核心因素。研究表明肠巨噬细胞活化引起的小肠平滑肌炎症是导致POI的关键因素,肠道菌群可通过IL-1R1/MyD88通路活化肠巨噬细胞而在POI中发挥重要作用。课题组前期研究发现POI小鼠Enterococcus丰度显著增加,参黄膏贴敷神阙穴在有效治疗POI的同时可调节肠道菌群。因此,课题组认为外科手术引起的菌群紊乱可通过IL-1R1/MyD88通路活化肠巨噬细胞而引发POI,参黄膏贴敷神阙穴可通过调节菌群组成及巨噬细胞活性而发挥疗效。课题组拟通过构建不同小鼠模型及基因编辑细胞,联合应用多种组学技术,从体内体外两方面深入研究肠道菌群Enterococcus介导的IL-1R1/MyD88活化在POI中的作用,从调节肠道微生态-免疫环路揭示参黄膏贴敷神阙穴的作用机制,为POI的中医外治法提供理论依据,为临床推广应用奠定基础。
英文摘要
Postoperative ileus (POI) is a common complication of iatrogenic abdominal surgery and a core tach in postoperative rehabilitation. It was reported that intestinal inflammation caused by muscularis macrophages is a key factor leading to POI and intestinal microbiome play a crucial role by activating muscularis macrophages via IL-R1/MyD88 pathway. Our previous results showed that the abundance of Enterococcus was significantly increased in mice model of POI and Shenhuang paste, the effective medicine for treating POI, could regulate the structure of gut flora. Therefore, we hypothesize that the dysbiosis of intestinal flora caused by surgery could result in POI by activating intestinal muscularis macrophages via IL-1R1/MyD88 pathway, and Shenhuang paste on Shenque point could exert its therapeutic effects by regulating the composition and function of intestinal flora, which thereby further regulating the activity of muscularis macrophages. For studying the role of Enterococcus-mediated activation of IL-1R1/MyD88 pathway in POI and revealing the efficacy mechanism of Shenhuang paste on Shenque point, we are going to carry out in vivo and in vitro experiments combined with multi-omics analyses. Our study provides theoretical basis for the treatment of POI with Chinese traditional methods and establishes foundation for the clinical application.
