慢性咳嗽是肺部手术后最常见并发症,严重影响患者生活质量。针刺肺经改善咳嗽作用确切。PKA/PKC敏化TRPV1在慢性咳嗽反射调节中具有重要作用。我们研究发现针刺肺经能改善肺部手术后慢性咳嗽,患者术后血液TRPV1水平较术前上调。预实验结果发现,豚鼠肺部手术后慢性咳嗽模型肺组织TRPV1表达上调,针刺干预可抑制该蛋白表达。因此我们推测针刺肺经治疗肺部手术后慢性咳嗽可能经PKA/PKC抑制TRPV1敏化,影响SP、CGRP、NKA等神经肽释放,从而改善咳嗽症状。本研究拟观察针刺肺经对豚鼠肺部手术后慢性咳嗽气道炎症及PGE2、BK和TRPV1敏化的影响;应用TRPV1基因敲除小鼠,探讨TRPV1在针刺肺经治疗肺部手术后慢性咳嗽中的作用;联合使用PKA、PKC抑制剂研究针刺肺经对豚鼠肺部手术后慢性咳嗽、肺组织及延髓中神经肽的调控机制。本研究为揭示针刺改善肺部手术后慢性咳嗽的临床应用提供实验依据。

Chronic cough is the most common complication after lung surgery, which seriously affects the quality of life of patients. Clinical studies have confirmed that acupuncture at lung meridian can improve the symptoms of cough. The sensitization of TRPV1 by PKA/PKC plays an important role in the regulation of chronic cough reflex. Our previous study found that acupuncture at lung meridian can improve the symptoms of chronic cough after lung surgery, and the level of TRPV1 expression in the postoperative blood is significantly higher than that before surgery. Pre-experimental results showed that the expression of TRPV1 in the lung tissue of guinea pig chronic cough model after lung surgery was up-regulated, and acupuncture intervention could inhibit the expression of this protein. Therefore, we hypothesized that acupuncture at lung meridian for chronic cough after lung surgery may inhibit the sensitization of TRPV1 by PKA/PKC, and affect the release of neuropeptides such as SP, CGRP and NKA, thereby improving the symptoms of cough. To confirm this hypothesis, this study is to observe the effects of the acupuncture at lung meridian for chronic cough airway inflammation and PGE2, BK and the sensitization of TRPV1 after guinea pig lung surgery. TRPV1 knockout mice were used to investigate the role of TRPV1 for acupuncture at lung meridian for chronic cough after lung surgery. Combined use of the inhibitor of PKA and PKC is to study the regulation mechanism of acupuncture at lung meridian for guinea pig chronic cough, and the neuropeptide of the lung tissue and medullary after lung surgery. This study provides an experimental basis for revealing the clinical application of acupuncture to improve chronic cough after lung surgery.
