颈动脉粥样硬化斑块破裂是导致脑梗死主要原因,探索斑块进展病生理机制,尽早发现易损斑块是国际研究热点。研究发现炎症与斑块发生、发展、破裂密切相关,但炎症过程及其与预后关系的病生理机制尚不清楚。新生血管和巨噬细胞是炎症重要组成部分,两者相互作用导致斑块进展。MRI和PET是评价颈动脉粥样硬化斑块有力手段,动态增强MRI能提供新生血管情况,(18)F-FDG-PET能提供炎症反应巨噬细胞代谢信息。因此本研究应用目前国际最前沿一体化PET/MR,将颈动脉粥样硬化患者分为无症状、症状性保守治疗、症状性手术治疗三组,进行斑块动态增强MRI和(18)F-FDG-PET同步扫描,分析形态结构、新生血管和代谢改变及相关性,并长期纵向随访,观察斑块动态演变情况,比较影像变化与临床指标和组织病理的关系,揭示斑块内新生血管和巨噬细胞的炎症特征,阐明炎症与斑块稳定性的病生理机制,进行早期影像学预警,有效预防脑梗死。

Rupture of carotid atherosclerotic plaque is the primary cause of cerebral infarction. It is a leading research area in atherosclerosis to investigate the pathophysiological mechanism of vulnerable plaque progression for early detection. Recent studies have shown that inflammation may cause the occurrence, development and rupture of atherosclerotic plaque, but its physiological mechanism and relationship with the outcome of atherosclerotic plaque are poorly understood. Neovascularization and macrophages are the important components of inflammation and their interaction leads to plaque progression. MRI and PET are the non-invasive imaging technology and increasingly used to evaluate atherosclerotic plaque. As dynamic enhanced MRI is used for imaging neovascularization with high spatial resolution, (18)F-FDG-PET can be used for quantitative imaging macrophages metabolism of response to inflammation with high sensitivity and specificity. In this study, patients with carotid atherosclerosis will be divided into three groups: asymptomatic, symptomatic conservative treatment, and symptomatic surgical treatment. We will use the most advanced simultaneous PET/MR to study morphological structure, neovascularization and metabolic changes in the 3-group patients with carotid atherosclerosis; to estimate the relationship between morphological structure, neovascularization and metabolic change; and to assess the outcomes during follow-up. The outcomes from the study will help us to reveal the inflammatory characteristics of neovascularization and macrophages in plaques and to understand the physiological mechanism of inflammation and plaque stability. In conclusion, our project will be quickly translated to clinical to provide early imaging warning of vulnerable plaques, and develop more effective treatment strategies. This will play an important role in effectively preventing the occurrence of cerebral infarction.
