Development of the care model for elderly with dementiaAging-in-place each type of community based care services

我们开发了一种护理模式，使患有痴呆症的老年人能够在每种类型的社区护理服务中继续住在家里或在熟悉的环境中（就地养老）。在社区养老服务中帮助老年痴呆患者，需要满足3个因素：(1)建立一个老年痴呆患者能够得到适当的医疗和临终关怀的体系；(2)为护理人员提供支援，让他们掌握适当照顾失智长者所需的知识和技能；(3)加强与当地重要人士和机构的合作。

We developed a model of care enabling elderly people with dementiacontinue to live at home or in familiar environment(Aging-in-place) in each type ofcommunity-based care services. To assist elderly people with dementia age-in-place atcommunity-based care services,3factors should be satisfied: (1) developing a system inwhich elderly people with dementia can receive proper medical care and end-of-life care;(2) supporting care staff so they can acquire the knowledge and skills necessary to provideproper care to elderly people with dementia; and (3) enhancing cooperation with keypersons and agencies in the local community.
