OBESITY'S EFFECT ON MYOCARDIAL METABOLISM AND IT'S REGULATION

This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Obesity is in epidemic proportions in the United States, affecting more than 34 million people. However, there is a paucity of data regarding obesity's effect on the heart, even though obese people have an increased risk of cardiovascular (CV) death. It is know that obesity, particularly upper body obesity, is associated with risk factors for CV disease and left ventricular (LV) structural abnormalities such as hypertrophy and failure; however , the effect of obesity on cardiac metabolic function is unknown. We hypothesize that since cardiac metabolism is abnormal in other conditions associated with LV hypertrophy and failure (e.g., hypertension), that cardiac metabolism will be abnormal in obese subjects. Thus, the specific aim of the pilot project is: To determine the effect of upper body obesity (i.e., a BMI>30 kg/m2 and a waist circumference> or = 35 inches in women and > or = 40 inches in men) on cardiac metabolism in humans, to relate this metabolism to the levels of substrate available for cardiac metabolism and circulating anabolic hormone levels, and to compare these values with those of lean subjects (BMI < 25kg/m2). In order to investigate this, we plan to recruit fourteen otherwise healthy, obese subjects. We will measure cardiac metabolic levels (i.e., medium-chain fatty acid, glucose, and lactate levels), cardiac size, percent body fat, and (using positron emission tomography or "PET" scans) determine cardiac blood flow, overall metabolic rate, and aerobic fatty acid metabolism in both groups. The cardiac metabolic rate and fatty acid metabolic rates will be analyzed in the context of the metabolic substrate. We will then compare the measured values of cardiac blood flow, metabolism , and fatty acid metabolism of these obese individuals with date from 14 age-matched, healthy lean (BMI<= 25) individuals that were studied under currently funded projects.
