Propylthiouracil (PTU) is a mainstay of medical treat-ment for hyperthyroidism during pregnancy. It inhibitS thyroid hormone synthesis, extrathyroidal conversion of thyroxine (T.) to 3, 5, 3'-triiodothyronine (Ta), and deiodinative degradation of the hormonally inactive T metab-olite 3, 3'5'-triiodothyronine (rT,), PTU Crosses the placenţa, however, and at high doses it may cause fetal goiter and hypothyroidism. Low doses of PTU (300 mg or less daily) are thought to afford satisfactory control of maternal hyperthyroidism without causing Clinically evi-dent thyroid dysfunction in the neonate. After encountering an infant with fetal goiter and hypothyroidism (index case)(the mother was receiving 400 mg of PTU daily), the authors initiated a prospective study of thyroid function at birth, at 1 day, and at 3 days in jnfants born to hyperthyroid mothers who had been treated with low doses of PTU. These infants, although clinically euthyroid, had biochemical evidence of mild, transjent hypothyroidiSm.Eleven women were treated for hyperthyroid Graves' disease during pregnancy with the smallest dose of PTU that could control their hyperthyroidism adequately. Seven women also received thyroid hormone supplemen-tatjorn in the form of| jotrix. Elood was taken from the umbilical cord and heel of each neonate. Forty cord serum samples from normal infants born during the study period served as concurrent controls. Normal values from the authors' records provided historical Controls.
