In 1930, Wiggers used high-speed cinematography to show that the heart passes through different stages during the first few tens of minutes as ventricular fibrillation (VF) continues. 1 Recently, it has become apparent that the best treatment during resuscitation following cardiac arrest is different at different times as the heart passes through these stages. Weisfeldt and Becker have proposed a three-phased model of cardiopulmonary resuscitation to reflect this time-sensitive progression of resuscitation physiology. 2 They call the first 4 minutes of VF the electrical phase in which the best treatment is immediate defibrillation. They call the period from approximately 4 minutes to approximately 10 minutes of VF the circulatory phase in which the best current therapy may be to delay defibrillation for 1–3 minutes, while chest compressions and ventilation are performed to provide oxygen to the heart. They label the period after approximately 10 minutes of VF the metabolic phase during which tissue injuries from global ischemic events and from reperfusion induces circulating metabolic factors that cause additional injury in addition to the effects of local ischemia. Weisfeldt and Becker review the evidence that cardiopulmonary bypass and external cooling may be beneficial during this phase but that epinephrine, which may be beneficial during the circulatory phase, may be detrimental in the metabolic phase. 2–7 Therefore, it is important to know the duration of VF before initiating the therapy during resuscitation.
