项目摘要

本研究は、子どもが生涯にわたる健康生活の基盤を形成するための施設である保育所配属の看護師・保育士のための、発達障害などの心の健康問題の把握とその支援のためのプログラムの開発を目指すものである。2021年度は、心の健康問題の構造化を目指し、発達障害等子どもの心の健康問題とその対処に関する先行研究の把握による実態調査を行った。その概要を、以下の2点にまとめる。まず、保育所における発達障害の子どもへの支援に関する研究動向と課題を明らかにすることを目的に、医学中央雑誌WEB版を用いて、「保育所」「発達障害」で検索を行った。104文献を対象として分析した結果、2005年の発達障害自立支援法制定後の2007年から研究への取り組みが大きく増加していることが分かった。支援内容は、発達障害の子どもと親だけでなく保育所も対象として報告されていた。他方、支援には課題も多く発達障害への差別や偏見があることが推察された。発達障害の子どもと親、保育所に加え、社会全体への啓発も含めた支援が不可欠であることが示唆されている。次に、保育所における発達障害が疑われる子どもへの早期介入の促進支援の現状を明らかにすることを目的に医学中央雑誌WEB版を用いて、「保育所」「発達障害」で検索を行った。抽出された中から35文献を分析対象とした。発達障害が疑われる子どもへは保育所の支援に加え、園医、保育所の看護師、保健師、乳幼児健診を行う医療施設、療育センター、心理士、作業療法士、特別支援学校等による支援と多岐に渡っていた。また、それぞれの保育所で支援が行われている現状も明らかになった。今後、すべての発達障害が疑われる子どもに早期介入が行われるためには、保育所と関連機関との連携方法や保育所への支援の在り方を検討する必要が示唆された。
