项目摘要

V180I遺伝性Creutzfeldt-Jakob病（CJD）症例の臨床症状を前方視的に検討し、初発症状、ミオクローヌスの出現時期と消失時期、脳波上の周期性同期性放電の出現時期と消失時期、無動性無言状態に至る時期、経管栄養施行の有無、および髄液検査所見、MRIにおける拡散強調像の高信号の変化や脳萎縮の進展を経時的に観察している。また、V180I遺伝性CJD症例の過去の剖検例についても、臨床症状、画像・検査所見の推移を後方視的に検討し、全経過や死亡原因も合わせて検討し、経管栄養を含めた対症療法の延命効果、プリオン病治療の可能性を示した。また、プリオン病治療薬の臨床治験における評価項目についての必要事項を明らかにした。現時点までに19例（平均発症年齢78.8歳、全経過平均46.3ヵ月）の臨床所見を検討した。全例でプリオン病の家族歴はなく、MM1型孤発性CJDと比べて、統計学的に有意に女性に多く、高齢発症で、長期経過を呈していた。脳波上の周期性同期性放電を認めた症例はなかった。ミオクローヌスは軽度である一方、驚愕反応が目立つ症例が多く、特徴的な症状として、病的笑いや顔面模倣が多くの例で認められた。11例で経管栄養が施行され、経管栄養を施行した症例の方が、施行しなかった症例に比べて統計学的に有意に長期生存していた。V180I遺伝性CJDの特徴的な自然経過を明らかにし、孤発性CJDとの違いを示した。これらのデータは診断基準や診療ガイドライン改訂への活用が期待され、生前診断の向上にもつながることが期待される。有効性が期待される抗プリオン病薬投与時には、治療効果判定の際に利用する基礎データとなることが期待される。
