本年度は、実験環境の整備と解析に向けての検体収集を進めた。また、既存の臨床データを収集し、その解析を行うことで、代表的な炎症性サイトカインであるInterleukin-6（IL-6）の、非高齢/高齢敗血症患者それぞれにおける予後との関連につき、検討を行った。動物実験では、ヒトに類似した加齢性変化を観察すべく、文献検索により、非高齢/高齢マウスそれぞれの、適切と考えられる週齢の決定を行った。結果、本研究では、非高齢マウスは16-20週齢、高齢マウスは88-104週齢を用いるのが適切と考えられた。また、高齢マウスは安定した供給が滞る可能性があるが、これらの週齢のマウスを安定供給可能な業者の選定も行った。敗血症作成のための手術手技（盲腸結紮穿孔 [CLP] 手術）は既に習得しているため、今後、これらのマウスを用いて、非高齢/高齢それぞれの敗血症モデルを作成し、血液検体の採取を行いサイトカインの解析に進む予定である。ヒト敗血症症例を対象とした解析の準備としては、千葉大学医学部附属病院ICUに入室した、敗血症患者の臨床データ、血液検体の収集を進めた。使用可能な既存検体も確認した結果、令和3年度末時点で、計500例の血液検体を蓄積できている（非高齢 [75歳未満] 331検体、高齢 [75歳以上] 169検体）。今後の解析に備え、引き続き検体収集を継続する。既存の敗血症患者の臨床データ（千葉大学医学部附属病院ICU入室症例、n=529）を用いた解析では、IL-6血中濃度は非高齢敗血症患者では生命転帰と関連する一方、高齢敗血症患者では、その関連が乏しいことを示す結果が得られた。これは、バイオマーカーとしてのIL-6の有用性に新たな知見を加えうると考えられる。今後解析を深め、学会・論文での発表を行う予定である。
