项目摘要

今年度は、テーマを2つ設定した。1つは、「薬局と地域包括支援センター（以下、センター）の連携による認知症早期発見プロジェクト」の構築とその有用性評価である。当初、薬局を起点とした、認知症の早期発見⇒センターへの連絡⇒センターによる介入⇒早期の診断・治療・支援という連携体制の構築を試みたが、最初から認知症に特化すると、相談事例の掘り起こしが難しいことが判明した。そこで、協力地域の薬局、行政、センター関係者と協議し、薬局を「高齢者の総合相談の窓口」として機能させ、センターと情報共有することにより、要援護高齢者に対する医療・介護の連携体制を構築し（「薬局・包括連携ねっと」と命名）、それを通して認知症への対応力を強化することにした。その結果、当該地域の10か所の薬局からセンターへ9例/年の介入依頼があり、うち4例が認知症事例であった。もう１つは、認知症患者を介護している家族介護者（以下、認知症家族介護者）を支援するスキルを薬局薬剤師が修得するための研修プログラム（START-PharmD)を開発し、その有用性を確認した。STARTは、英国ロンドン大学で開発された、認知症の周辺症状BPSDへの対応方法、患者への上手な接し方、介護者自身のストレスマネジメントなど、認知行動療法をベースにした認知症家族介護者への支援プログラムである。START-PharmD:START for PHARMacist to support Dementia patient and caregiver」は、STARTを参考に、薬局や在宅で薬剤師が服薬指導や相談対応時に、介護者の心理的な不安や負担を軽減できるようなコミュニケーションスキルを学ぶ。参加者は4回コースで18名、うち研修前後のアンケートデータが揃っている6名について、研修の有用性を検証した結果、いずれも他者へ受講を推薦するなど高い評価を得た。
