65歳以上の高齢者は全がん罹患の約75％を占める。高齢者の意思決定の特徴として、治療の判断を主治医にゆだねがちで自身の治療に対する価値や希望の表出が少ないという課題がある。高齢がん患者の価値観や希望の表出促し、よりよい協働意思決定を支援する解決方法の開発が期待されるが、わが国では高齢がん患者の診療時のコミュニケーションの特徴すら十分明らかではない。本研究では、高齢がん患者の診療における患者と医療者の意思決定への認識、意思決定行動、医療者の共感的行動、および関連する医学・社会的要因を明らかにすることを目的としている。これまで、高齢がん患者の意思決定に関連する文献レビューを行った。また高齢者の治療の意思決定時の医療者との診察時のコミュニケーションを明らかにするため、先行調査の診察録音データの二次分析を進めている。国内で高齢者がん患者の意思決定に関する調査研究は14本と少なく、分析に耐える情報が限られたが、高齢がん患者特有の意思決定の認識、行動、家族や医療者との関係要因、社会・医学的要因について、研究者間で議論しながら高齢者の診療の場における協働意思決定モデルの概要を確認した。
