项目摘要

本研究の目的は、がん薬物療法を受ける患者の治療継続とQOL向上を目指し、多職種連携による「患者を支える医療」の具体的なモデルケースを検討することである。これまでに作成した「処方別がん薬物療法説明書」は、病院で運用中で、Webサイトでも公開（https://www.scchr.jp/information-prescription.html）している。本年度は、直接、患者から、説明書の分かりやすさ、患者・家族が必要とする情報、医療者との関わりについて意見を収集することを計画した。複数診療科でアンケートとインタビュー調査を実施中で、予定数58人のうち2023年3月末時点で14人が終了した。まだ少数の結果だが、「説明書は役に立つと思いますか」、「副作用の対処をしやすくしたと思いますか」という設問に対して肯定的な回答のみであった。一方、インタビューでは、患者・家族から「症状があったとき、いつ、どの程度になったら病院に連絡するのか迷う」、「どこに連絡すればよいか分からない」などの意見があったため、2023年4月に説明書に掲載している連絡先を表紙に移動し、約150種の全ての説明書の更新を行った。説明書は、患者・家族の知識の獲得・補強と、医療者への報告の判断を助け、患者の治療継続とQOL向上に寄与するための情報支援ツールと位置づけている。さらに、行動支援として、患者が副作用を自己管理するツールを検討してきたが、患者・家族の意見から、病院へ連絡するかどうかの判断を補助するツールの必要性が見出せた。そこで、いくつかの症状について、Yes/Noチャートのようなツールを多職種で検討し、Webサイトに掲載する方針とした。
