项目摘要

本研究は被験者が抱く「治療であるという誤解」について、それが生じる可能性を最小限にすること及び、臨床研究における最善のインフォームド・コンセントのあり方を提案することを目的としている。本研究はこれまで、「治療であるという誤解」は被験者のインフォームド・コンセントにおいて何が問題となるのか、について検討した。治療とは当然ながら患者当人の利益である疾病の治癒や症状の緩和であるが、研究は患者（被験者）個人ではなく科学的知見の獲得を目指している。被験者が治療と研究を区別せず、研究参加を継続することで被験者は研究では原則的に期待できない治療効果に対するニーズを持つ。また医療者はこのような被験者の期待に対して適切に応答しないことは、研究者としても医療者としてもその義務と責任を果たしていない。「治療であるという誤解」に関しては、研究者が被験者数を獲得したいために無意識に「治療であるという誤解」を誘発しやすい言動や態度をとっている可能性がある。研究者が研究者と治療者との役割の違いを自覚した上で、どこまでが治療でどこからが研究なのかについて確実に被験者に説明と理解を得る必要である。そしてこのような取り組みについて本邦ではどこまで行われているのかについては調査・報告がなく、明らかでない。本研究は臨床試験のうち、がん臨床試験における治療であるという誤解に焦点を当てている。その理由はがんという疾患の特徴から患者の治療への期待が高いこと、そしてこのことは治療であるという誤解がある程度は避けられないことを意味するからである。がん臨床試験医参加する被験者は治療であるという誤解を抱いて当然の状況にある。このような被験者の心理を汲み取り、適切なインフォームド・コンセント取得に加え、研究参加時の心理的サポート体制を検討する必要性がある。
