前年度の調査結果を検討した結果、今年度は看護職者を中心とした服薬に関する心理教育プログラムを神奈川県下の私立精神病院急性期治療病棟において試験的に実践した。対象者は、精神分裂病と診断されすでに病名の告知を受けており、急性期の幻覚や妄想などの症状が激しくなく退院に向けての自立援助が中心で、あらかじめこのプログラムの趣旨を説明し賛同を得られた者とした。また、1セッションを約60分、計4回とし、第1回〜2回は主に疾患や薬物療法の理解を促進するための教育セッション、第3回〜4回を主にこれまでの病気や薬に対する認識や感情を表出し今後の生活に向けた希望や展望を語る対応セッションとした。参加者は4名(途中1名が病状悪化のため中止)であった。参加者の言動や反応を毎回フィールドノートに記録するとともに、第4回が終了した時点でアンケートを行い、プログラムの評価を行った。結果は、(1)従来の服薬指導や医師による病状説明とは異なり、参加者の不満や疑問などが表現されやすく、疾患や薬物療法の理解が促進する。(2)同じ障害を持つ者同士が感情や体験を共有することで、エンパワメントされやすく、自ら疾患をコントロールする意志を強化できる。(3)看護職者は、参加者の理解度や予測される問題を把握し、問題解決を援助しやすくなる。などの効果があることがわかった。また、今回のプログラムが精神分裂病患者のみを対象としたことから、他の精神疾患(特に躁鬱病)のプログラムや患者本人のみならず家族を対象としたプログラムを構築する、回数や技法(たとえば視覚教材の開発)・他職者との連携方法などを洗練する、プログラムを終了者の疾患再発や服薬行動の変化などの経過を調査することで効果を検証する、などの必要性が示唆された。
