项目摘要

脳卒中患者(以下,患者と略す)の就労支援における患者・家族,企業,地域関係機関・職種のネットワーク構築のためにモデル地域(以下,地域と略す)を決め、その地域でのネットワーク(以下,ネットワークと略す)を構築することとした。地域の患者の就労支援経験のある事業所担当者(回復期リハ病棟SW,ケアマネジャー,訪問リハ,障害者活動支援センター相談員,研究者)で6回検討会を行い、就労支援の現状と課題を共有した。課題として、回復期リハ病棟側は「在宅復帰が目的のため就労支援につながりにくい」「退院時に障害の程度に応じた連携先が様々で連携ができにくい」「職場との連絡調整方法論がない」、地域の事業所側は「患者・家族の障害理解の程度」「介護保険担当者に就労支援の経験がない」「就労支援に報酬がない」「医療機関との連携がとりづらい」「雇用側の状況の影響」だった。就労支援対象者を障害・症状別に4つ(主に身体障害・主に高次脳機能障害・身体障害と高次脳機能障害・介護保険や障害者手帳なしの軽度後遺症)に分類した。ネットワークとして患者・家族,企業,地域関係機関・職種が患者の就業能力を同じように理解・把握できるための情報シート(以下,情報シートと略す)の作成、4つの障害・症状別の就労支援のフローチャートを作成することとした。情報シートは現在、作成中である。情報シートは基本情報、後遺症、復職等への患者の思い、復職に向けて利用できる制度及び企業の支援状況、就業能力評価は業務遂行の行為ごとに評価できるものにしている。評価者は患者・企業・就労支援者とし、評価時期は回復期病棟退院時、その後は復職等まで定期的に行うこととした。就労支援開始時期は回復期リハ病棟入院時からとし、情報シートは患者が保持し支援担当や企業に渡すこととした。患者が地域で就労準備として利用できる社会資源も検討中である。
