项目摘要

令和4年度はコロナ禍再拡大のため、前向きRCT研究である「術前化学療法を施行する切除可能境界膵癌患者に対する術前PROリハビリテーションの効果」を検証するための事前研究として、「切除可能境界膵癌に対して術前化学療法を施行した患者の術前身体機能に関する後ろ向き観察研究」を継続することにし、症例を集積して調査を進めた。令和4年度は、広島大学病院で切除術を施行された切除境界型膵癌患者で、術前化学療法を行った21名（男性15名、女性6名）を対象として調査した。電子カルテから術前化学療法前後の握力（kg）と６分間歩行距離（m）、併存疾患を後ろ向きに調査した。統計学的解析は、術前化学療法前後の各項目の比較に対応のあるt検定を用いた。握力（前：27.0±8.0 kg、後：27.5±7.4 kg）と6分間歩行距離（前：456.3±93.8 m、後：485.0±78.7 m）は術前化学療法前後で有意な差を認めなかった。しかし、どちらかが低下した者は７名、どちらも低下したものは5名であり、この12名は共通して、糖尿病、高血圧、脂質異常症を含む高血圧性疾患を併存していた。切除境界型膵癌に対し術前化学療法の有効性が示されている一方で、身体活動量の減少に伴う身体機能低下が危惧される。本研究結果からも、患者の併存疾患を考慮し、術前化学療法中のリハビリテーション治療により身体機能を維持する必要性が示唆された。尚、本研究内容は第6回日本リハビリテーション医学会秋季学術集会で発表した。

In 2010 and 2014, the Department of Health conducted a pilot study to investigate the effects of preoperative chemotherapy on patients with cancer and the effects of preoperative chemotherapy on patients with cancer. 21 patients (15 males and 6 females) undergoing resection and preoperative chemotherapy at Hiroshima University Hospital were investigated in 2004. The effectiveness of preoperative chemotherapy for resection of boundary cancer is shown in one side, and the reduction of physical activity is accompanied by the risk of low physical function. The results of this study demonstrate the need for maintenance of physical function in patients with comorbidities and preoperative chemotherapy. The content of this study was presented at the 6th autumn academic meeting of the Japanese Medical Association.
