慢性疼痛の高い罹患率は本邦含め世界的に問題となっており、特に高齢者の占める割合は大きい。慢性疼痛患者は自分の痛みが周囲に理解されないことに対する不満や悲観を抱えており、それらに基づく認知や行動パターンは疼痛の難治化の要因となっている。認知行動療法（Cognitive Behavioral Therapy CBT）はそうした認知や行動から形成される悪循環の改善を目的とする心理療法であり、疼痛治療におけるエビデンスが蓄積されつつあるものの、効果は小から中程度にとどまる。一般に就労をしていない高齢患者は家族と過ごす時間が長く、慢性疼痛患者とその家族は痛みの訴えを理解されない患者も、訴えを聴き続ける家族も相互に負担となる。本研究は慢性疼痛患者とその家族に対し、週1回８０分のオンライン認知行動療法を全１０回実施した群と、通常診療群を比較し、疼痛による日常生活障害度や疼痛強度、不適応的感情などの包括的な痛み指標の変化について検証することを目的とした。リクルートは千葉大病院の認知行動療法センターを中心に行い、当院整形外科、麻酔科、歯科口腔外科、総合診療科、等の紹介によるものであった。本年度までの全登録者数は８名（介入群４名、対照群４名）であり、対照群に対しては待機期間終了後レスキュー試験を行った。現在７名の介入が終了しており、2023年8月までにすべての介入が終了予定である。目標症例数は３０名（介入群１５名、対照群１５名）であったが、研究初診を千葉大病院で行う点でコロナ禍の影響をうけ、リクルートが滞った。そこで研究効率を鑑み、最終年度前年度申請した結果、発展研究が承認されたため、本研究は現段階で終了とすることとした。本研究はパイロット研究であり、両群の質的検証は可能なデータが取得できたため、従来の解析方法とは変更となるものの質的な観点から結果を解析する予定である。
