咀嚼能力とフレイルの関連にサルコペニア肥満は媒介するのか～中間媒介分析を用いて～

本研究期間は2020年4月から2023年3月までとなっている．しかしながら，本研究課題の元となる大迫検診がCOVID-19の影響で2020年度と2021年度が全て中止となり，データベースの拡充が行えなかった．加えて，研究代表者が2021年9月から産休・育休に入り研究を中断し，そのまま退職する運びとなったため，現在も研究を中断している状態が続いている．今回解析可能なデータセットである2019年度1年分での予備解析を行ったため，その結果を報告する．研究開始時には咀嚼能力の低下は，サルコペニア肥満を介してフレイルと関連するのかを地域一般住民を対象に中間媒介分析を用いて横断的に明らかにすることを目的としていたが，解析対象者の人数が少ないため，今回は地域在住高齢者における咀嚼能力と関連する指標とフレイル指標との関連を横断的に調査することを目的とした．検診項目は，咀嚼能力の指標として現在歯数，最大咬合力および咀嚼能率の測定をし，フレイル指標としては握力および歩行速度を用いた．65歳以上の地域高齢者67名を解析したところ，現在歯数20歯未満，最大咬合力および咀嚼能率と歩行速度の低下に有意な関連は認められなかった．また，現在歯数20歯未満，最大咬合力および咀嚼能率の低下と握力の低下に有意な関連は認められなかった．今回の解析では地域在住高齢者における咀嚼機能とフレイルの関連は認められなかった．検診対象者が健康意識の高い高齢者が多く含まれていたことや，サンプルサイズの不足も要因として考えられる．今後の継続的な検診の実施によってサンプルサイズの拡大を目指し，豊富な交絡因子を組み込んだうえで解析を行っていきたいと考えていた．また，研究開始当初予定していたサルコペニア肥満の中間媒介分析も行っていきたいと考えていたが，そのまま退職する運びとなり研究を継続が困難となり今回廃止申請を行った．

The diagnostic items include the index of masticatory ability, the number of teeth present, the maximum bite force and the masticatory energy rate, the index of grip strength and the walking speed. 67 people over the age of 65 years old have the highest geographical height, and the number of teeth present is 20 years old. The maximum bite force and the chewing energy rate are lower. The maximum bite force and masticatory energy are lower than the grip strength when the tooth number is 20. The analysis of the present situation is that the region is living in a high place, and the chewing function is related to the recognition. The health consciousness of the patient is high, and the patient is high. In the future, the implementation of the diagnosis of the disease will be carried out in a timely manner. The study was initially scheduled to begin and the intermediate media analysis was conducted.
