项目摘要

本研究は、自然災害により自宅を失い、応急仮設住宅（以下、仮設住宅とする）で暮らす循環器疾患をもつ人のセルフモニタリングパターンを明らかにし、災害支援活動を担う看護職者が循環器疾患をもつ人のセルフケアの自立を目指して行う、看護援助モデルの開発を目指す研究プロセスの一部である。研究協力者9名の災害後の時間経過に伴う暮らしの変化の時期毎に、セルフモニタリングのタイプに分類し、3つのセルフモニタリングのパターン：「関心事解決による取り戻しパターン」「生活再建に伴い取り戻しつつあるパターン」「喪失後の新手段獲得が必要なパターン」を抽出した。2022年度は、セルフモニタリングのタイプ及び、各パターンの課題を見出し、研究結果および文献検討から必要な看護援助を検討した。災害後の循環器疾患をもつ人のセルフモニタリングを高める看護援助として、＜優先される関心事の解決を図る看護＞、＜疎かになったセルフモニタリングを取り戻す看護＞、＜喪失したセルフモニタリングを取り戻す看護＞、＜医療を継続する看護＞を見出した。災害による循環器疾患をもつ人のセルフモニタリングのパターンは、災害によって影響を受けたセルフモニタリングのタイプの変化と病状の経過から、帰結が良好であったパターンを目指す必要がある。日本では高齢化や循環器疾患患者の増加が進んでおり、自然災害による中長期的な健康被害が明らかになっている。そのような中で、被災し応急仮設住宅で暮らす循環器疾患をもつ人のセルフモニタリングの特徴に着目し、その特徴から必要な看護援助を見出したことは、災害後の循環器疾患をもつ人の症状や病状の悪化に早期に気づく対処する手立てとなる。また、被災地で活動する看護職者が人的・物的・時間的資源が限られる中で、循環器疾患をもつ人のセルフモニタリングのタイプのアセスメント視点が明確になり、必要な看護援助を検討することができる。

The viewpoint of the caregiver is clear, and the necessary nursing assistance is discussed.
