本研究の目的は、心血管系の手術を受ける高齢患者に対する看護師のフットケアの実態と、周術期の足の形態および機能を経時的に明らかにすることである。【研究１（令和２～３年度）】心臓血管外科病棟における看護師のフットケアの実施状況を調査し、今後の課題について検討した。大学病院2施設の心臓血管外科病棟の看護師に郵送式質問紙調査を実施した。フットケアの実施状況は、「足の状態の観察」と「保湿」が80％以上の実施率あり、実施率により分けた2群と看護師の経験年数5年未満と5年以上の2群間による有意な差は認めなかった。足の観察の内容としては、「皮膚の色調」、「創傷の有無」、「浮腫の有無」は90％以上を占めていたが、「胼胝鶏眼の有無」「感染徴候」「末梢の血行動態」「足の形状」については10％以下であった。心臓血管外科病棟の看護師のフットケアの実態について、足の「観察」の実施率は高かったが、主に下肢の術後に関連した「浮腫の有無」や「色調」の観察であり、生活習慣から起こる足トラブルに着目した観察やフットケアはほとんど行われていないことが明らかになった。【研究２（令和４～５年度）】心血管系の手術を受ける患者における術前後の足症状および機能の調査のため、倫理審査に申請中である。倫理審査申請前には、対象者の基準および情報収集項目を検討するため、実際の心血管系の術後の患者のリハビリの様子を見学し研究計画書の準備を進めた。倫理審査の承認後は、速やかに臨床でのデータ測定を進めていく。
