術後心房細動は、術前・術中・術後に存在する複数の要因が影響し合って発症すると報告されているが、それらのリスクファクターを有していてもなお、臨床現場では術後心房細動を発症する患者と発症しない患者がおり、個体差が認められたため、いまだ検証されていない他の要因も関連しているのではないかと推測された。そこで今回、術後心房細動発症の大きな素因の可能性がある自律神経活動のバランス、とくに術前の活動レベルに着目した。本研究の目的は、心臓血管外科手術施行後に高頻度で生じる術後心房細動の新規発症を予測するため、術前の自律神経活動レベルが術後心房細動発症に関連しているかどうかを検証し、さらにどのような自律神経活動バランスが術後心房細動の高リスクとなるか明らかにすることである。心臓血管外科手術施行予定患者の術前心電図で心房細動や頻発する不整脈が認められないこと、ペースメーカを装着していないこと、維持透析を行っていないこと等を適格基準とし、症例登録を行った。2022年度は、研究開始から3年目となり、初年度から継続して患者への説明同意後にデータ収集を行っており、登録症例は130例程度に達している。一方、COVID-19パンデミックの影響も継続したため、症例の登録が困難な状況は続いた。2022年12月で新規症例の登録を終了とし、解析段階へと移行し、解析時データの収集を行っている。これまでに本研究実施に伴う有害事象は発生しておらず、安全に試験を実施できた。
