本研究課題は、訪問看護師が主導する慢性疾患患者の在宅エンドオブライフケアプログラムの開発を目的としており、以下の手順で進めている。第1段階は、訪問看護師を対象としたインタビュー調査により、人生の最終段階にある慢性疾患（がん・非がん）の在宅療養者と家族に対する訪問看護師の支援内容および実践上の困難を明らかにすることである。第2段階は、インタビュー調査の結果から、訪問看護師が「人生の最終段階の医療・ケアの話し合い」を実施するタイミングを評価するための尺度を作成することである。第1段階の結果、23名の訪問看護師のインタビュー調査を終えた。分析の結果、訪問看護師が実施する「人生の最終段階の医療・ケアの話し合い」のタイミングに共通性がみられた。がん疾患では、訪問看護師は、患者の健康状態やケアニーズの変化（患者の全人的苦痛の増加）、日々のケアによる家族介護者の心身の状態の変化（家族介護者の苦痛の増加）、患者が辿る終末期のプロセス（疾患の軌跡）を踏まえて、終末期の話し合いのタイミングを特定していた。非がん疾患では、症状の悪化、患者や家族介護者のエンドオブライフへの意識の欠如、日常生活動作の低下のタイミングで、話し合いを実施していることが明らかとなった。第2段階では、インタビュー調査の結果から、全34項目からなる「人生の最終段階の医療・ケアの話し合い」のタイミング評価尺度を作成した。予備調査の結果、有用な質問項目を選定し、最終的に、がん疾患では21項目、非がん疾患では23項目となった。現在、全国の訪問看護師を対象に、本調査を実施している。
