高齢者の転倒の原因は、身体機能障害や能力低下を含む内的要因、環境要因を含む外的要因など多岐に渡る。また本人の行動要因、転倒に対する恐怖心や自信度も、影響因子であることが明らかになっている。そのため転倒予防介入に関して近年では、特定の要因に特化した運動介入のみよりも、教育指導・リスク評価などとの併用の有用性がいわれている。中でも介入視点として、転ばない自信度を示す転倒予防自己効力感、身体能力の自己認識と実際の能力の乖離に起因する転倒リスクが注目されてきている。したがって、高齢者自身が転倒に関わる要因を適切に認識して対応できることが転倒予防には重要であり、それらを支援する工夫・仕組みの開発が課題になると考える。先行研究でもこれらの転倒リスクについて言及されているが、相互の転倒への影響度や関連性、また介入効果までは、異なる見解も示唆され十分に明らかになっていない。本研究では、高齢者の転倒予防におけるより効果的なリハビリテーションの確立を目指すにあたり、まず転倒要因として、転倒予防自己効力感と、自身の身体能力の自己認識（過小・過大評価）との関連を明らかにすることを目的とした。2022年度は、機能障害を呈するデイケア利用の地在住高齢者を対象とし、研究協力施設の病院の協力のもとで横断的調査を行った。その結果、転倒予防自己効力感の低さと身体能力の自己認識の乖離の大きさは、非転倒群に比べ転倒群において身体機能低下の程度に関わらず、転倒と高い関連性があることが明らかになった。また、身体能力に関する自己認識の実際の能力との乖離は、転倒の発生に対してより説明力の高い因子であることが示唆された。さらに本研究の参加者のように、すでに何らかの障害を持ち、支援を受けながら日常生活を送っている高齢者は、身体能力を過大評価する傾向が認められた。
