本研究は、転倒予防自己効力感を高めるプログラムを作成し、それを大腿骨頸部骨折など、転倒に伴う骨折の既往のある転倒恐怖高齢者に適用し、その効果を明らかにすることを目的とした。転倒予防自己効力感を高める介入プログラムとは、自己効力感を高める4つの情報源(制御体験、代理体験、社会的説得、生理的・感情的状態の自覚)に着目し、それぞれの情報源から、歩行指導、バランス・筋力強化を目的とする運動指導、生活指導、話し合い、モデル高齢者の紹介等を実施するものであった。このようなアプローチを総合病院で実施されている転倒予防教室に採り入れ、2002年9月から2003年9月までの教室終了者を転倒骨折群(43名、平均年齢80・7±3.5歳)とその他の対照群(117名、平均年齢80.7±9.4歳)に分け、プログラム開始時および終了時の転倒予防自己効力感(FPSE)と移動能力の変化を比較した。FPSEは本研究のために独自に開発した尺度で、布団の出入り、更衣、簡単な食事の支度を転ばずに、やり遂げる自信を問う身辺動作SE、階段を下りる、混雑した場所・不安定な床面・薄暗い場所の歩行、両手に物を持っての歩行を転ばずにやり遂げる自信を問う移動SEからなる。転倒予防教室開始時の年齢、FPSE、移動能力について、転倒骨折群と対照群の間に有意な群間差は見られなかった。転倒予防教室の開始時と終了時の比較では、転倒骨折群は移動能力に有意な変化がみられなかったものの、移動SEが有意に向上し(p<0.05)、階段を下りる、不安定な床面を歩くなどのバランスを崩しやすい移動を転ばずにやり遂げる自信が向上したことが示された。その一方で、対照群は移動能力が改善したものの、FPSEに有意な変化は見られなかった。転倒予防自己効力感を高める介入プログラムは、転倒によって骨折した経験を持つ地域高齢者に対して有効であることがうかがわれた。

The purpose of this study is to prevent the occurrence of high stress and to make it clear that high stress is the cause of fracture of the femoral neck and that high stress is the cause of fracture. Comparison between the beginning and the end of the fall prevention class, the fall fracture group, the ability to move intentionally, the movement SE intentionally upward (p<0.05), the stage downward, the unstable bed surface, the movement of the fall prevention class, the self-confidence upward. The ability to move in one direction and in the opposite direction is improved. FPSE is intentionally changed. To prevent self-intervention, we should pay attention to the prevention and treatment of fracture.
