项目摘要

住民ベースのがん登録のデータに基づき、偏りのない患者の抽出をすることが本研究の主目的の一つであったため、全国がん登録の公表値及び院内がん登録の0年集計に基づいて、東京都及び大阪府における協力医療機関候補の上部消化管がんの患者数を算出した。2019年の全国がん罹患数によれば、食道（C15）男21,719、女4,663、胃（C16）男85,325、女38,994、であり、国立がん研究センター中央病院C15：501、C16：740、東京都立駒込病院C15：253、C16：358、大阪国際がんセンターC15：456、C16：675、大阪病院C15:34、C16：177であった。このうち、プロトコルに従って150例をリクルートする計画を立てた。共同研究相手であるオランダ国立がんセンター（IKNL）から博士課程学生を招聘し、全国がん登録情報に基づいて国立がん研究センター中央病院の電子カルテシステムから医療情報（EHR）を抽出するパイロット研究を実施した。食道がんまたは胃がんと診断された患者を、日本（2016-2019年）またはオランダ（2015-2020年）の情報を抽出し、年齢、性別、ステージ、治療、化学療法のレジメン等に基づき生存率をKaplan-Meier法を用いて算出した。結果は英文論文として投稿中である。また、令和3年度から繰り越した研究費に基づいて、患者報告アウトカムシステム（ePRO）を利用した長期フォローアップのシステムを作成した。具体的には、①疾患特性：がん登録データよるがん情報と医療記録からの補完、②患者の属性：性別、生年月日、居住地、併存疾患、および全身状態、③治療、ケアパターン、④行動・生活習慣因子：喫煙、アルコール摂取量、身体活動、BMI等、⑤生活の質：QOL（EORTC-QLQ-C30、QLQ-OG25）を継続的かつ電子的に患者が入力するものである。
