乳房再建術は乳癌患者数の増加とともに世界的に急速に増加しているが、その半数以上に術後痛が発生し、患者の生活の質を落とし、社会復帰への支障になって いる。本研究は、従来の患者申告の主観的アンケート調査(生活の質と術後痛)に加え、客観的評価(術前の内因性鎮痛と術前後の機能的脳画像解析)を加える ことにより、術後痛発生因子を確立することを目的とする。また術後痛の詳細な部位別調査も加える。 痛みを客観視化する手法として、1内因性鎮痛(endogenous analgesia: EA)測定、2脳機能画像の二つがある。1内因性鎮痛は、ヒトの持つ生理的な「痛み の修飾調整」(conditionedpain modulation)を測定することにより、対象者がもつ内因性鎮痛能力(EA値)を数値化する手法である。2脳機能画像は、頭部の 磁気共鳴イメージング(magnetic resonance imaging:MRI)のことであり、一般的にヒトを対象にしたマクロレベルでの脳科学研究として、脳形態、機能、機能 的ネットワークを用いた解析手法である。予定乳房再建術の周術期における客観的指標を取得し、主観的評価との相関解析することにより、術後痛発生因子を確 立する。われわれの確立している客観的評価とは、1内因性鎮痛測定と2脳機能画像解析であり、主観的評価とは、患者申告型のアンケート(BREST-Q日本版 と、部位別痛みの術後調査票)である。本研究では乳房一次再建30例を対象として、乳房再建前後の過程で生じる、生活の質(BRAST-Q)の調査の患者主観的変 化を、再建術前と術後(術後3ヵ月時以降)と比較することに取り組んでいる。痛みに関しては術後翌日の急性期と術後(術後3ヵ月時以降)を調査する方針と なっている。

Breast reconstruction has led to a rapid increase in the number of breast cancer patients worldwide, with more than half of them suffering from postoperative pain, a decline in the quality of life of patients, and obstacles to social recovery. This study aims to investigate the main factors (quality of life and postoperative pain) reported by patients, and to establish the main factors (quality of life and postoperative pain) reported by patients.
