敗血症治療の血液浄化療法に関しては，non-renal indicationで行われることは推奨されておらず，海外での研究もわずかである．また海外での持続血液浄化法としてはContinuous venovenous hemofiltration(CVVH)が基本であり，高流量のCVVHでサイトカイン濃度(IL-6)を低下させた研究があるが（2006,GhaniRA），死亡率や重症度スコア，腎呼吸器間の短縮を認めた研究はない．日本においては，保険診療上1日の浄化量に限りがあるため，透析を加えたCHDFがしばしば行われている．本邦では2014年7月に重症敗血症もしくは敗血症性ショックを適応としたCHDFの膜が上市 された，AN69ST膜は治験において，サイトカイン吸着性能が認められ(Shiga et al.Blood Purif 2014;38:211-8)，製造販売承認となった．ただShigeらの論文においてもAN69ST膜を用 いて治療した敗血症患者34名での検討であり，ほかのCHDF膜やCHDFを行う群と行わない 群との比較ではない．本邦発の敗血症治療としてアピールしていくためには質の高いエビデ ンスは必須であり，多施設での研究が必要になるだろう．サイトカインの除去と病態の改善 の関係性に関しても賛否両論あり，この辺りを明らかにするための第一段階の研究ととらえている。現在までにICUに入室した敗血症性ショックの患者に対して，AN69ST膜による持続血液ろ過透析を行った症例と，その他の種類の透析膜を使用した敗血症性ショック患者の経過に関して解析を行うべく，症例を収集している．2022年度もCOVIDー19患者の治療に精力的にかかわっており、通常の医療業務を圧迫している状況もあって，思うような患者数を蓄積できていないのが現状である．引き続きデータ収集を行っていきたい．
