项目摘要

コロナ禍における東京都緊急事態宣言などの影響から被験者の確保が難しい状況となっていた（2020年4月～）。本研究における被験者の年齢層が高齢者であることから研究協力依頼に関しては慎重に取り組む必要があると考えていた。現状、感染症法に基づくと新型コロナウイルス感染症の位置づけは、これまで、「新型インフルエンザ等感染症（いわゆる２類相当）」とされていたが、令和５年５月８日から「５類感染症」へと変わった。これらの経緯から本研究の課題として被験者の確保があるが、デザイン、対象者、測定方法について再度吟味し、実現性の面から妥協すべき点や計画の実行可能性について共同研究者とも検討し判断する。本研究の中間解析結果から無歯顎者においてTCIと義歯満足度およびOHRQoL、グミゼリーにて測定された咀嚼能力と主観的咀嚼能力およびOHRQoLに有意な相関が認められた。このことから、TCIと咀嚼能力の改善がOHRQoL改善に寄与する可能性が推測された。ここで、単相関を用いて確認された係数の有意性は交絡の管理がなされていないという問題があるため、中間解析に用いられた観測変数に加えて、無歯顎者においては顎堤条件や口腔乾燥などの口腔内環境および維持安定や適正な顎間関係付与などの義歯の質、有歯顎者においては歯周組織評価や残根などの残存歯の質および数を観測変数として解析に適用する必要が出てくる可能性があり検討を要すると考える。本研究における年齢層が高齢者であり、意思の疎通が難しい場合があり、意思疎通困難者に対して適応可能な検査法を今後考案する必要性を感じている。
