项目摘要

抗凝固薬電子患者日誌は、経口抗凝固薬であるアピキサバン、エドキサバン、ダビガトラン、リバーロキサバン、ワルファリンを内服している患者向けに服薬の記録と副作用の早期発見目的に開発した。本電子患者日誌は患者自身のスマートフォンやタブレットを用いて、WEBからアクセスするシステムであり、受診の記録、日々の記録、記録を見る画面から構成される。患者は本システムにアクセスし、服薬の有無、服薬できなかった場合はその理由を入力し、その日の体調、血圧、脈拍、体重、出血の有無についても記録する。東京理科大学薬学部にて、画面設計を行い、システム会社に開発を依頼した。本電子患者日誌を活用した服薬アドヒアランス向上を検証する臨床試験を計画した。対象は経口抗凝固薬を服用中で、抗凝固薬電子患者日誌を使用でき、同意を得られた患者100名であり、２カ月間の試験期間のうち、１カ月間は抗凝固薬電子患者日誌を使用し、１カ月間は使用せずに通常の服薬管理を行う、ランダム化クロスオーバー試験とした。倫理審査委員会で承認を受けたのち、順天堂大学医学部附属順天堂医院循環器内科及び聖路加国際病院循環器内科の２施設で臨床試験を実施した。主要評価項目は服薬達成率（服薬達成率＝（観察期間－飲み忘れの日数）/観察期間x100）とし、副次評価項目は薬剤毎の服薬達成率、副作用の発生状況、患者の抗凝固薬電子患者日誌に対する使用感、医療者に対する抗凝固薬電子患者日誌の有用性の意識調査とした。得られた研究成果は第87回日本循環器学会学術集会チーム医療セッションシンポジウム等で報告した。

The electronic patient diary for anti-coagulation drugs, including oral anti-coagulation drugs such as Apiki Saban, Edeki Saban, Dvitralin, Rilaroki Saban, and Walfu Saban, was developed for the purpose of recording and early detection of side effects of the drugs taken by patients. This electronic patient diary consists of the patient's own records, daily records, and screen views. The patient's body tone, blood pressure, pulse, weight, and presence or absence of bleeding were recorded. For example, 100 patients were enrolled in the anti-coagulation drug administration, the anti-coagulation drug electronic patient diary was used, consent was obtained, the trial period was 2 months, the anti-coagulation drug electronic patient diary was used in 1 month, the general service management was conducted, and the trial period was 1 month. The Ethics Committee acknowledges that the Department of Circulatory Medicine, Suntendo Hospital, Faculty of Medicine, Suntendo University and the Department of Circulatory Medicine, St. Luke's International Hospital have conducted clinical trials. The main evaluation items include the service achievement rate (service achievement rate =(observation period-number of days of drinking)/observation period x100), the secondary evaluation items include the service achievement rate of each item, the occurrence status of side effects, the user perception of anti-coagulation electronic patient diary for patients, and the awareness survey of the usefulness of anti-coagulation electronic patient diary for medical practitioners. The results of the research were reported at the 87th Japan Society of Circulators Academic Conference on Medical Science and Technology.
