【目的】患者家族とのオンライン相談の有用性と限界について、従来の対面式相談と比較して明らかにすること。【研究デザイン・セッティング・対象者】日本の教育型急性期病院における2020年4月1日から2021年9月30日までの前向きコホート研究。対象は20歳以上の入院患者とその家族のうち、同意を得て、入院期間中に家族と医師とのオンラインまたは対面による医療相談が実施された患者を対象とした。【主要評価項目】主要評価項目は、医療相談の対象（病状と治療計画、生命に関わる事象への対応方針、退院後の支援）を評価した。副次的評価項目として、受診回数を評価した。【主な結果】オンライン相談は58名の患者家族に、従来の対面相談は53名の患者家族に実施された。オンライン相談群で複数回の相談を受けた患者のうち、46名（79％）が対面相談も受けた。また、相談内容については、オンライン相談、対面相談ともに100％の患者家族が病状や治療方針について相談を受けていた。生命の危機に対する方針については、オンライン相談群では47%の患者家族が相談を受けているのに対し、対面相談群では53%であった。退院後のサポートについては、オンライン相談グループの59％が相談しているのに対して、対面相談グループでは40％であった。オンライン相談グループ58名のうち、オンライン相談95名、対面相談93名、合計188名の相談が行われた。生命の危機に対する方針に関する相談は、対面相談が有意に多かった（p<0.05）。退院後の支援については、オンライン相談の方が対面相談よりも有意に多かった（p＜0.05）。相談に参加した家族の人数は、オンライン相談の方が有意に多かった（p＜0.05）。
