项目摘要

スピリチュアルペイン（本研究ではスピリチュアルペインを「人間存在の根源的問題に触れる痛み」とする。以下、SP）に関する研究は、これまでスピリチュアルケアが主に終末期の患者に行われてきたこともあり、認知症高齢者のSPに関する研究は少ない。さらに認知症高齢者へのコミュニケーション法は欧米からもたらされており、日本発のものはほぼ見受けられない現状にある。そこで本研究は、まず、認知症による行動心理症状（Behavioral and Psychological Symptom of Dementia.以下、BPSD）とSPの関連について検証し、認知症高齢者のSPに関するアセスメントシートを開発・検証していく。そして、本研究は、認知症高齢者のSPを緩和する、簡便で日本の風土や習慣にあったコミュニケーション法を開発・実施・検証を行うことを通して、より質の高い認知症ケアを可能にする汎用性のあるコミュニケーションモデルの開発を目指している。本研究は５ヵ年計画であり、初年度は、認知症高齢者のBPSDとSPの関連を検証すべく、高齢者施設において認知症高齢者のBPSDが表出された場面(８ケース)を録画し、当該データを文字化、分析を行った。２年目は、認知症高齢者のSPに関するアセスメントシートの開発に向けて検討を行い、村田氏等の理論をもとに開発された「SpiPas」を根拠とし、特定場面のSPの可能性を蓄積した。さらに、蓄積した特定場面におけるSPの可能性について、エリクソンの発達課題やマズローの欲求段階等の理論を根拠とし、総合評価として示すアセスメントシートを開発した。３年目となる令和４年度は、これまでに収集した事例２件について、開発したアセスメントシートの試行を試みた。また、アセスメントシートを試行した結果については、２学会にて発表を行った（10.研究成果　学会発表参照）。
