项目摘要

本研究は、膵癌術後患者を対象にストレス応答の中枢を担うとされるCorticotropin-Releasing Hormone (CRH)系分子の腫瘍内発現と癌の進行や情動、Quality of Life (QOL)との関連を検証し、腫瘍に対する中枢神経系の介在可能性を腫瘍生物学・精神腫瘍学的に解明することを目的としている。先行研究において、CRHが癌増殖に関与する可能性が報告されている。一方QOLは近年、がんの生存期間に影響する因子としても注目されている。QOLの構成要素でもある抑うつは、ストレス反応に伴う情動でありCRHとも関連深い。CRH receptor 1の刺激は不安を惹起することが既に明らかにされており、疾患の進行とそれに伴う情動に対しCRHの関与が示唆される。以上の点から、癌患者の抑うつは、一般に考えられている、病気をストレッサーとして認知した結果生じる精神的反応である側面以外に、腫瘍による生体侵襲が直接脳にシグナル伝達された結果生じる生理的反応の側面がある可能性が考えられる。そこで本研究では、膵癌患者に対する前向きコホート研究によって腫瘍組織におけるCRH系分子の発現、病状、ストレス関連生理学的指標、患者報告によるQOLや抑うつ等のストレス関連指標を評価し、膵癌患者において疾患の進行と抑うつやQOLの間に中枢神経系の介在が存在し得るか検討を行う。調査は膵癌外科治療のハイボリュームセンターで実施し、膵癌患者に対しガイドラインに沿った十分な診療体制が整った条件のもとでデータを集積する。令和4年度は、令和3年度までに登録した被験者の追跡調査を実施し、併せて引き続き適格基準を満たす被験者の登録を行った。また、膵癌過去症例をもとにCRHと予後に関する解析結果を国際誌に論文発表した。
