好酸球性鼻副鼻腔炎は慢性鼻副鼻腔炎の中のサブタイプの一つであるが、喘息を合併することが多いという特徴を持つ上に、非常に難治性である。また、アレルギー性鼻炎は非常に患者数が多く、特にスギ花粉症は国民病とも言われている。　これら２つの疾患は、上気道に好酸球性炎症をきたす疾患である。これらの疾患は、患者の生活の質を下げるだけでなく、労働生産性の低下などをもたらすことが明らかとなり、社会的にも大きな問題となっている。これら２つの疾患の病態形成において、サイトカインやケモカインなどの炎症因子が重要な役割を持つことが明らかとなっている。これらの免疫関連タンパクは互いに相互作用を持ちつ つ、全体として各疾患の難治化に深く関わっている。また、プロスタグランディンも炎症因子の一つとして重要である。本研究では、好酸球性鼻副鼻腔炎症例やアレルギー性鼻炎患者を対象に組織中の各種サイトカイン、ケモカイン、 および、それらの受容体、また、各種プロスタグランディン関連因子の遺伝子発現を検討してきた。好酸球性鼻副鼻腔炎については、本邦から診断基準の提案が行なわれており、それにもとづいて重症度分類を行った。その結果、中等症・重症の好酸球性鼻副鼻腔炎症例は、非好酸球性鼻副鼻腔炎症例や軽症の好酸球性鼻副鼻腔炎症例と比較して組織中の各種炎症因子の遺伝子発現が高値であった。また、好酸球性鼻副鼻腔炎症例の中で、喘息合併症例や鼻副鼻腔手術後の再発症例は、コントロール群と比較して組織中の各種炎症因子の遺伝子発現が高値であった。昨年度は新型コロナウイルス感染症の影響もあ り、研究の進行が順調ではなかったが、これまでの結果から、サイトカインやケモカイン、プロスタグランディンを制御してゆくことがアレルギー性鼻炎や好酸球性鼻副鼻腔炎の病態解明や治療に有効であることが示唆され、今後、引き続き検討を続けてゆく予定である。
