项目摘要

本研究の目的は、地域の高齢者が通院する中規模病院の外来看護師が在宅療養支援を行うために必要な看護実践能力の自己評価尺度を開発することである。令和元年は、第1研究となる、外来看護師における高齢者に対する在宅療養支援に必要な看護実践能力の項目（案）を先行文献を参考に作成し、200床未満の病院10施設の中で同意の得られた外来看護師20人を対象に項目の調査を行った。令和2年度と3年度は第2研究を行った．第2研究の目的は，令和元年に明らかになった中規模病院の外来看護師における高齢者に対する在宅療養支援の看護実践能力の因子構造の妥当性と信頼性を明らかにし尺度を作成した。令和4年度は、第2研究の論文執筆を行った。論文投稿後、査読を受け修正を行った。その結果、在宅療養支援の看護実践能力の自己評価尺度から、役割認知尺度の命名が適切であると判断した。そのため、量的研究者の指導のもと、再度、探索的因子分析と確証的因子分析を行った。分析の結果は、外来看護師234人（回収率47.2％）から回答を得たデータを用い、項目分析後に、20項目を探索的因子分析し、3因子15項目が抽出した。因子名を【予測アセスメント役割】、【リレーションシップ役割】、【コミュニケーション役割】と命名した。Cronbach’s α係数は尺度全体で0.922と高い信頼性を示した。また、確証的因子分析においても、適合度指標は、GFI＝0.889、AGFI＝0.850、CFI=0.942、RMSEA＝0.072であり、構成概念妥当性が確認された。論文掲載の見込みがあるため、尺度の実用性の検討を行うために、インタビュー調査の研究計画書を作成し、調査を開始している。
