项目摘要

令和４年度の研究成果は以下の通りである。第１に、アメリカにおける２０１０年患者保護アフォーダブルケア法（Patient Protection and Affordable Care Act of 2010；通称「オバマケア（Obamacare）」）成立後の州政府による医療保障政策の展開過程を、無保険車（uninsured）が多くそれらの人口比が州の中で最も高いテキサス州における医療扶助のメディケイド（Medicaid）改革をめぐる州・連邦政府間関係の検討を通して実証的に明らかにした。この研究は、後述の現地調査における州政府の医療保障政策担当部長および地域医療連携担当ディレクターへの聞き取り調査を交えて行った。研究成果として、『テキサス州のメディケイド1115ウェイバーをめぐる州・連邦政府間関係の動態：連邦政策の動向を見据えた州政策の展開』と題する学術論文をまとめることができた（前年度に入稿済および掲載決定・今年度に若干の加筆修正を経て上梓）。第２に、テキサス州において現地調査を行い、医療保障政策における地域医療連携戦略および政策過程にかかわる専門家への聞き取り調査を行うとともに、それらの訪問機関や州内の医療機関等において資料収集を行った。第３に、次年度に実施予定のテキサス州およびワシントンD.C.における現地調査の準備として、これまでに入手した文献・資料・統計等の分析を行うとともに、海外の研究協力者との連絡調整を行った。
