项目摘要

本研究は小児看護専門看護師が行う高度実践看護に注目し、「医療的ケアを必要とする子どもと家族へのケアとキュアを融合した高度実践看護ケアガイドライン」の開発を目的としている。令和4 （2022）年度は、医療的ケアを必要とする子どもと家族へのケアとキュアを融合した小児看護CNSの高度実践看護の範例事例の解釈と各ケースから導き出されるサブテーマの分析の洗練化を行い、その記述と文献検討を行いながら考察を進めた。必要な医療的ケアを確実に実施できる子どもと家族の支援体制をつくり、子どもが生きていく力をつけながら活動の幅を広げ、子どもと家族の成長発達を見据えたケアとキュアを融合した高度実践看護が見いだされている。オンラインでの専門学会の学術集会や研修会への参加を行い、医療的ケアを必要とする子どもと家族、高度実践看護、研究手法の情報を収集した。その結果、現象学的アプローチ、事例研究の方法、専門看護師の実践に関する新しい知見および子どもが病院や自宅以外で社会生活を安全に送るための切れ目のない支援の必要性、成長期にある子どもの病態や症状の変化に応じた介護や治療、子どもの成人への移行支援等の課題が見いだされた。現在、考察の視点とケアとキュアを融合した高度実践看護の記述を熟読して、より深い文脈から見えてくる記述の洗練化を進めている。本研究の新規性と学術的独自性である、人々の間で何か気づかれているがまだはっきりとみることができない高度実践看護、小児看護専門看護師の実践知について明らかにして、ケアガイドラインの開発の着手につなげていく。
