项目摘要

本研究の目的は、糖尿病を有する消化器がん患者に対して、[1] PRO(patient reported outcome)を電子的に評価するシステムの開発および実行可能性の検討と、[2] 周術期から療養期の一連の過程におけるPROの経時的変化を検討することである。高血糖との関連性が示唆されている胃がんと大腸がんで手術療法を受ける者を対象とした。しかし、昨年度に続きCOVID-19の蔓延下にあり術後の患者への直接的な接触が制限されたため、目的[2]を実行するだけの対象者の確保は非常に困難であった。そのため、目的[1]の電子的PRO評価システムの検討を中心に行った。2回目の試験的調査を来年度に実施することを目標として、本年度はそのための準備を行った。1回目の試験的調査で抽出された課題に対するwebシステムの改善については、システム仕様の大きな変更が難しく、時間的制約もあることからQOL・PRO尺度を見直し質問数を減らすにとどまった。定期的なPRO測定に関するwebシステムへのアクセス作業の効率化（測定時期にメール受信し、添付されたURLからwebシステムへアクセス）については、将来的にアプリケーション化するなど簡便化に向けた検討が必要である。また、1回目の試験的調査において参加者の離脱が多く、その理由の一つとしてPRO評価システムへのアクセス方法など説明・指導不足が考えられた。そのため、COVID19等の感染症や個人的事情により指導する機会が制限されるケースを想定して、オンラインを活用するなどの指導方法を検討した。さらに、配布資料の簡略化についても再考した。今後は、本年度に検討したPRO評価システムや調査のプロトコール、指導ツールを用いて試験的調査を行い、PRO評価システムを用いた定期的なPRO測定の実行可能性について検討していく必要がある。

This paper discusses the guidance methods for the control of infectious diseases such as COVID19 and personal affairs. In addition, the distribution data is simplified and re-examined. In the future, this year's review will focus on the investigation of PRO evaluation, guidance and use of PRO evaluation, and the possibility of implementation of PRO evaluation.
