本研究の目的は、婦人科がんサバイバーに対する療養支援の向上を目指し、[1]定期的にPROを測定・収集できる電子版PRO (ePRO)評価システムの検討と、[2]療養期における婦人科がんサバイバーのPRO（症状・機能・QOL）の時間的変化に関連する影響要因を探索的に検討することである。対象とするがん種の選定については、対象施設が保有する院内がん登録情報（2016～2020年）より、婦人科がんに該当するICD-O-3（国際疾病分類―腫瘍学第3版）の部位コード及び形態コードをキーとして症例を抽出し、がん種別に症例数と平均年齢(±標準偏差)を割り出した。その結果、子宮頚部が946例（年齢50.1±15.1歳）と最も多く、次いで卵巣225例（年齢60.2±16.8歳）、子宮体部153例（年齢65.6±13.9歳）であった。解析可能な症例数を満たしており、いずれも療養期において経過観察を要するがん種であることから、この3つを対象とすることに決定した。PRO項目の選定については、昨年度に引き続き先行文献および電子カルテより得られた診療情報をもとに退院後の療養期において測定すべき項目を検討した。特に性生活などセンシティブな内容に関する質問項目については、先行文献から十分な情報が得られなかったことから、電子カルテ上の診療録や看護記録等のテキスト情報を用いて、テキストマイニング手法により特徴的なワード抽出が可能か分析を実施している。今後はテキストマイニングの解析結果も含めて、療養期における支援構築のために重要なPRO項目を最終化し、電子端末でPRO測定できるアプリケーション化の実現可能性を含めて検討していく必要がある。
