本年度においては、回我々は剖検例を用いて、癌関連VTEの病理組織学的特徴を解析した。宮崎大学医学部附属病院の連続剖検症例 (n=2,564) より、VTE症例 (n=180) を抽出し、担癌VTE (n=114)と非担癌VTE (n=66)に分類し、臨床病理学的背景について比較を行った。血栓内癌細胞の有無、血栓関連因子の発現、血栓組成（赤血球、血小板、フィブリン、好中球外細胞トラップ）、器質化の程度について、VTE組織の病理組織学的解析を行った。癌関連VTEでは、27%の症例で血栓に癌細胞が確認され、深部静脈や肺動脈への浸潤、あるいは小集簇性に癌が存在していた。また血栓内癌細胞の41%に腫瘍壊死を認めた。免疫組織化学では、血栓内癌細胞の88%に組織因子あるいはポドプラニンの発現を認めた。好中球細胞外トラップは、初期の器質化血栓で発現が認められ、非担癌VTEとの発現差は明らかではなかった。これらの結果から、血栓内癌細胞が凝固因子や血小板凝集因子を発現することで、血栓形成を促進し、癌関連VTEの病態に寄与することが示唆された。本研究は、多数症例において、人体病理に基づいた癌関連血栓症の特徴を明らかとした初の研究である。本研究成果は、国内学会発表、及び国際英文雑誌に掲載された (Gi T, Kuwahara A, Yamashita A, et al. This year, we analyzed the histopathological features of carcinoma-related VTE. A total of 2,564 cases of VTE (n = 180) were diagnosed at Miyazaki University School of Medicine, and 114 cases of VTE (n = 114) were classified as malignant and 66 cases of VTE (n = 66) as nonmalignant. The presence or absence of cancer cells in thrombus, the development of thrombosis-related factors, the composition of thrombus (red blood cells, platelets, leukocytes, and extracellular cells), the degree of organization, and the pathological analysis of VTE tissues were investigated. Cancer-related VTE was identified in 27% of cases, with thrombus and cancer cells infiltrating deep veins and pulmonary arteries, and small clusters of cancer were present. 41% of the cancer cells in the thrombus died. Immunohistochemistry showed that 88% of the tumor cells in the thrombus were tissue factor positive. Good globular extracellular matrix, early organic thrombosis, non-cancerous VTE, poor development These results indicate that clotting factors and platelet aggregation factors are present in cancer cells within the thrombus, that thrombosis is promoted, and that the pathogenesis of cancer-related VTE is mediated. This study is the first study on the characteristics of carcinoma-related thrombosis in most cases and in human pathology.
